Specific Due Dates for Forms

Form 1 - Registration Form – due March 31 (prior to year of participation)
Form 2 - Athletic Participation Form – due August 1
Form 3 - Official Game Schedule – due one week before first game
Form 5 - Official Team Eligibility List – due one week before first game
Form 7 - Liability Waiver – schools keep on file for every student/athlete
Form 8 - Agreement and Support Clause – due prior to each season
Form 11 - Officials' Association – due at the beginning of the school year
Form 13 - Tournament Team Roster – due two weeks before state tournament
Pre-participation Physical Evaluation – schools keep on file for every student/athlete
